This notice applies to anyone receiving care in the programs, services and activities at Advocate Health. This notice applies to all people in our facilities.
Advocate Health:
- Offers free aids and services to people with disabilities to communicate with us, such as, but not limited to:
- Qualified sign language interpreters
- Written information in other formats (large print, audio, accessible electronic formats, other formats)
- Provides free language services to people who do not speak English as their first language. These services include:
- Qualified medical interpreters
- Information translated into other languages
- Does not ask people to use minor children, family members, or friends as interpreters.
If you need these services at an Advocate Health location, tell a staff member.
How to File a Service Complaint
You can file a complaint in person, by mail, or email if you believe that Advocate Health did not provide these services or if we treated you unfairly (discriminated). If you need help filing a complaint, please speak with a staff member:
