Patient Financial & Billing Information

Harbin Clinic is committed to delivering quality medical care in Northwest Georgia. Our physicians are well-trained, use state-of-the-art technologies and offer a broad range of healthcare services.

We consider it a privilege and a responsibility to serve you and your family. However, it is your responsibility to make sure payment is made for all services. We will file your claims with your insurance company and make every effort to collect the appropriate payment from them based on your benefit coverage. It is your responsibility to pay any co-payments, co-insurance, deductibles or benefits not covered by your insurance company.

Once your insurance company has processed your claim you will be issued a statement. Your payment for the balance is expected upon receipt of the statement. If you are unable to make the full payment, it is important that you contact our Business Office at 888-427-2461 to make arrangements.

PATIENT RESPONSIBILITIES

  • Present your current insurance card(s) at each visit. Failure to do so may result in your having full responsibility for your bill.
  • Payment of copay and/or any patient responsibility is required at the time services are rendered.
  • Notify us promptly if you discover that registration errors have occurred. Most insurance companies have a limited time frame for filing claims.
  • Respond immediately to any received correspondence from your insurance company or from Harbin Clinic to ensure your claim is paid promptly.
  • Make any additional payments owed prior to the due date on your billing statement. Failure to do so may result in further action being taken on your account.

INSURANCE PLAN PARTICIPATION

Harbin Clinic participates in many healthcare plans. It is your responsibility to make sure that each physician you receive treatment from is a part of your insurance plan. In the event that a service requires any referrals or authorizations, it is your responsibility to confirm with your provider that any required referrals or authorizations have been obtained prior to seeking treatment from a Harbin Clinic physician.

HMO/POS/PPO PATIENTS

If your insurance company is one of our HMO/POS/PPO contracts, please present your insurance card and any necessary referrals at the time of service. It is your responsibility to pay any co-payments, co-insurance, deductibles or benefits not covered by your insurance company. Please be prepared to make payments at the time of service. Any specific benefit questions you have should be addressed to your insurance company.

INDEMNITY INSURANCE

(All other Major Medical Plans)

If you provide insurance information at registration, we will be glad to file claims for service with your insurance company. It is your responsibility to follow up with your insurance company on the status of your claims. If your insurance company does not make timely payment, you will be expected to make the full payment at that time.

MEDICARE PATIENTS

As a Medicare provider, Harbin Clinic accepts assignment from Medicare. If you have other supplemental insurance, we will file your claims for you. If you do not have other supplemental insurance, you will need to pay for the part that Medicare does not pay at the time of service. Any services that are not a covered benefit and are not paid by Medicare are also your responsibility.

MEDICAID PATIENTS

Harbin Clinic participates in the Medicaid program as well as some CMO’s. If you are enrolled in one of these programs or have applied for one of these programs, please inform the office staff at the time of your visit. If you are approved for Medicaid at a later date and qualify for previous services, you must immediately contact our Business Office. We can then file for any services that may apply. If you have a co-payment under the Medicaid program, it is due at the time of service.

WORKERS’ COMPENSATION

If your care is covered by workers’ compensation, your employer must call to schedule your initial appointment and to verify coverage before we can begin treatment. If your employer does not verify coverage, we cannot guarantee that your services will be covered under workers’ compensation. Should your services be denied by the workers’ compensation plan, you will be responsible for full payment of all services.

 

MOTOR VEHICLE ACCIDENT

If you are injured in a motor vehicle accident and you believe a third party is responsible for your care, we will assist you in filing claims for your healthcare services. However, if prompt payment is not received from the third party or legal statement, you will be responsible for full payment.

 

REGISTRATION PROCESS

First-time patients are required to complete a registration form and to provide current insurance card(s) and photo ID. If you do not have a current insurance card available, the full payment is due at the time of service. Established patients should be prepared to present insurance card(s) and photo ID at each visit and to update their personal information if there have been any changes, such as an address or phone number, since their last visit.

NO INSURANCE

If you do not have any insurance coverage, the full payment is expected at the time of service. If you are unable to pay in full, you need to notify the office staff prior to treatment to make arrangements for payment

 

METHODS OF PAYMENT

We accept cash, check, debit and credit card payments with Visa, MasterCard, Discover and American Express.

 

PROBLEMS WITH INSURANCE

Although in some cases we may have a contractual agreement with your insurance company, our relationship is with you, not your insurance carrier/company. If you feel your insurance did not pay as expected, you should contact them to resolve your concerns.
