Contract Summary

Term Of Employment: Two year contract - May be eligible to become a shareholder after two years
Compensation: Two year guaranteed salary - Bonus as approved by the Board of Managers in an amount equal to 50% of Physician's cumulative positive net reserve in year one and 75% in year two.

Vacation: Five weeks per year to include vacation and personal time
Medical Office: Clinic will assist Physician in furnishing consultation room. Medical equipment provided by the Clinic with allocation of expense to be determined
Agreements by Physician: Maintain professional associations and certifications
Licensed to practice in the State of Georgia
Expected to obtain Board Certification 
Hospital Privileges
Restrictive Covenant: For a period of one year after termination of Contract for any reason, Physician shall not practice medicine within a radius of thirty miles of Clinic's practice site

Benefits Summary

Health Insurance: Group Health Insurance Plan
  • Coverage becomes effective the first of the month following date of employment
  • Premiums are payroll deducted from gross pay before taxes
Life Insurance: Harbin Clinic provides all full-time physicians with a $50,000 life insurance policy which becomes effective their first day of employment. The premium is assessed through the physicians direct expenses at a group discount.
Long-Term Disability Insurance: All full-time physicians are eligible for long-term disability insurance coverage. The coverage becomes effective the first day of employment. Should a physician become totally disabled, the insurance would pay 60% of their salary up to a $25,000 monthly maximum to age 65. There is a 90 day waiting period before any benefits are paid. The premium is assessed through the physicians direct expenses at a group discount.
Savings & Profit Sharing Plan Upon completion of one year of service you are eligible to participate in the Harbin Clinic Profit Sharing Plan. Enrollment dates are January 1, April 1, July 1 and October 1 with a six year vesting schedule.
